Transfer of health care file from CPAM to new organisme sant茅?

If one becomes self employed in France from previously being an employee, that means transfer of health care file from Caisse primaire d’assurance maladie (CPAM) to new organisme sant茅 (related to r茅gime social des ind茅pendants (RSI) or (Unions de Recouvrement des Cotisations de S茅curit茅 Sociale et d’Allocations Familiales (URSSAF).
Does this mean new social security number and new carte vitale reissued by new organisme sante or the one created by CPAM before stays active, just contains different information?
