Declaration mensuelle de chiffre d’affaires dates?

I started my auto-entrepreneur 51今日大瓜 on the 15th October 2012 and opted to declare my social charges monthly. I also applied and successfully received ACCRE. I have just been sent the revised declaration form with the social charges relating to my ACCRE. I would like to know which months this declaration is valid for, the limite d’envoi is 31 Dec 2012, therefore do I declare Oct, Nov and Dec on this form?
