Creating a personal account with RAM to declare doctor

Hi Val茅rie, Hi Forum members,
I have received a letter from RSI/la Ram, Bienvenue 脿 la RAM. In this letter, they indicate that I need cr茅er votre compte en ligne personnalis茅...” and to enter my RIB pour le versement rapide de vos remboursements….
Also they have included a form with the letter, the D茅claration de m茅decin traitant form, although there is no mention of the form in their letter.
I have a couple of questions:
- Do I have to complete the d茅claration de m茅decin traitant form again? I have already done this in the past for the CPAM and already have a Carte Vitale - however, in the original declaration, I was the 产茅苍茅蹿颈肠颈补颈谤别 under my husband.
- Is it usual practice to create a personal account on ?
Just wanted to check these two points with you before I go ahead.
